Experience Verification Forms

You must provide experience verification with your application if you are applying for your license by exam. Verification of experience must be provided or signed off by a licensed plumbing or specialty contractor (or equivalent if from another state) whose supervision you worked under. You cannot provide your own experience verification. The below form(s) can be submitted if previous employers have not already provided experience verification documentation to you.

Annual Specialty License Renewal Form

Your renewal will be processed after we receive your payment. If you submit your renewal form online and select check or money order as your payment method, please be sure to reference your license number and mail your payment to:
