| Temporary cosmetologist, nail technician or esthetician license | $6 |
| Instructor initial license and renewal license | $25 |
| Reciprocity, initial license and waiver of examination | $100 |
| Certification or Endorsement | $20 |
| Apprentice license | $25 |
| Apprentice Salon initial license and renewal license | $250 |
| School initial license and renewal license | $300 |
| Student license | $6 |
| Salon or booth initial permit license | $60 |
| Salon or booth renewal | $35 |
| Reinspection of failed salon | $50 each reinspection |
| License duplicate or replacement | $5 |
